Full-Time Intake Coordinator – Hospice Agency
Seeking a reliable and experienced Intake Coordinator to join our team full-time. The ideal candidate must be organized, detail-oriented, and capable of handling multiple responsibilities in a fast-paced environment.

Responsibilities include:

Managing patient intakes and initiating services
Coordinating staffing and caregiver assignments
Following up on patient progress and care documentation
Posting clinical notes and maintaining accurate records

Must be available to work Monday through Friday, 10:00 A.M. – 4:00 P.M.
Must speak fluent English
Must have at least 1 year of experience working in a home health setting
Must be proficient in using Synergy (Home Health Software)
