Description

Schedule home health staff to see patients. QA medical records. Follow up with physicians and other stakeholders. Care coordination/transition of care with families and patients. Interaction with vendors, lab companies, insurance companies, and other stakeholders. Registering new patients into our EMR system. Communicating with families and caregivers for our patients. Communicating with the nursing team and home health aids. Full-time position ONLY! Candidate must be detail-oriented, able to multitask, and willing to work in a dynamic environment where a team approach is key. Must speak fluent English and have strong English communication skills. Must have strong communication skills as this position requires patient care coordination with other physicians, home health agencies, assisted living facilities, pharmacies, labs etc. Great customer service and phone skills with the ability to troubleshoot issues as they arise. Must have at least one year of office/medical experience
